Koko’s Japanese Restaurant Menu with Prices 9950 Jones Bridge Rd #500, Alpharetta

Did not demonstrate active managerial control measures are not satisfactory for establishment to be in compliance. Observed expired certification. Advised PIC that an update certificate is needed. Employees were observed washing hands with gloves on and without soap. Advised PIC that employees must discard gloves and wash hands properly using soap and warm water. COS- employees washed hands properly. Employee observed picking up shrimp from floor and did not wash hands. Advised PIC that hands are to be washed when changing task or they become contaminated. COS- employee washed hands. Observed food container and strainer stored in hand sink near grill. Hand sink must remain accessible at all times. No hand soap observed at hand sink near sushi prep area. Advised PIC that soap must be provided to properly wash hands. COS: soap was provided. No paper towels provided at hand sink near soup and white rice. Advised PIC that paper towels. COS: paper towels were provided. Observed in several units raw animal foods stored next to ready to eat foods. Advised PIC on the proper way to separate and store foods. COS: food was located and stored properly. Employee observed using single use gloves to pick up shrimp from floor. Employee used the same gloves to prepare sushi. Informed PIC of proper glove usage. COS: sushi and gloves were discarded. Observed employee wash dishes without using soap. Advised PIC that a cleaning agent is needed. Advised PIC of the proper way to wash dishes wash-rinse- sanitize. COS: cleaning agent was added to the first sink. Observed employee on prep line clean knife at hand sink without properly washing or sanitizing. Advised PIC of proper ways to wash and sanitize utensils. COS: knife was cleaned and sanitized properly. Microwave observed soiled. Advised PIC that microwave must be kept clean due to it is a food contact surface. Advised PIC to clean microwave frequently. COS: microwave was cleaned. Did not observed time marking for sushi rice. Advised PIC of that sushi rice must be time marked and discarded after 4 hours. COS: time mark was provided. Observed wiping cloths stored on the prep counters. Advised PIC that all wiping cloths not in use must be placed in an active sanitizing solution. (COS): Placed in solution.

Smokin Swamp BBQ – Menu with Prices – 4901-4953 NE Jacksonville Rd, Ocala

Only open a few months, these are kind of the new kids on the food truck scene. As assumed by their name they serve all types of smoked BBQ. Recently I enjoyed their brisket sandwich. The bun was fresh and the brisket was okay. Juicy but w/an odd texture and no ring. I know it was smoked w/hickory but if I didn’t know better I would think it had been boiled. I know, that’s funny…but I can’t explain it any other way. Nice size slices and a generous portion, it deserves 4 stars. They served 2 homemade sauces with it. One a sweet and tangy sauce and the other a mustard based vinegar sauce. Both were excellent. I’ve never been served by a food truck that added tax to their menu price but this one does. It makes it inconvenient for the customer to go digging for extra change when they don’t expect to. And it’s also time consuming for the owner to make change when normally you have the $ ready and “move them out quickly”. But, such is life. Mmmpa Lumpia – Menu with Prices – 4505 SE 13th St, Ocala

Lumpia anyone? You’ll find all types of it here from savory to sweet. Most of the time you’ll find them parked across from Jervey Gantt Park but they also can be found about town. The savory lumpia comes with a choice of 3 different sauces. Their “secret” sauce is sour cream based w/cilantro in it. The bourbon bbq is self explanatory and the sweet Asian is a teriyaki & soy. You can choose from a “meal” which includes 3 lumpia, fried rice & slaw. Or, you can purchase the savory lumpias individually at $2 a piece. I ordered the “slugworth” lumpias. More to the point it was chopped Mahi w/slaw & cheese. The only thing I could detect though was the taste of the Mahi & a few pieces of shredded cabbage. It lacked seasoning. Thankfully the “secret” sauce took care of some of the blandness. I’m looking forward to trying some of the other odd named lumpias in the future. Abby Singer’s Bistro – Menu with Prices – 617 Texas St, Shreveport

I can’t explain it! Well, this was my first experience at this restaurant and I wasn’t impressed. First off it was lunchtime and my time was limited. I waited 8mins at the wait to be seated sign for the rude waitress that coughed over everything to acknowledge I wanted service. It took a lady to walk over to her and tell her I was there… I ask the rude waitress who was the lady who alerted her of my presence and the waitress said” I don’t know who in the fuck she is”. Wow, what great customer service. I was told to sit at the bar to make my order. I ordered and watch the waitress cough, cough, touch money, cough into her hands and cough, cough, and cough without washing her hand. She then complain about her $3 tips. She cough and waited on other tables. This is how the flu and other germs are passed around. The Managers really need to teach your waitress hand sanitizing and how important it is to practice safe serve. I Ordered the chicken and waffles, it wasn’t a half of chicken and it was bland but with the hot honey sauce made it taste better. I wanted to take my family back for the experiences but I wasn’t really moved by the whole ideal….

Midnight Taco – Menu with Prices – 339 N Gilbert Rd, Gilbert

The food was pretty good not the worst I’ve had in Arizona but also not the best! I got a burrito. The reason for the lack of stars is because they seem to be very dirty and have little respect for health codes. While sitting there waiting to order, I watched the cook do at least 5 different things that are health violations. There was no hand washing site available in sight so I can’t imagine they wash their hands before handing food. If you are okay with people being gross while handling youre food, then you’re fine to eat here but if you’re like me and notice everything health related, it will make you sick to your stomach… The cook was sitting on a dirty bucket texting on his phone (he scratched at his nose and head a couple times) and after I order he popped up, threw some gloves on and started touching everything. Just because you put gloves on doesn’t mean you’re being clean…. I tried this place to try it, but I can say with certainty that I’ll never be back

Hibachi House – Menu with Prices – 937 Main St, Columbia

I watched the cashier take my card, touch the public pens that customers use to sign for credit card transactions, then go drop my spring rolls WITHOUT washing her hands or even using hand sanitizer!!!! THEN she went to scoop chicken onto the grill with those same unwashed hands!!!! That’s when the cook came from the back and took over. No, he didn’t wash his hands either! She STILL didn’t wash her hands but wiped her hands on a cloth they were using to clean the counter and cooking area with before going back to taking more orders. The cook used the chicken scooping spoon that she used to drop more chicken on the grill and then proceeds to cook food. He also touched the vegetables with his BARE hands without ever washing his hands as the water at the sink WITH soap was running yet never used it to wash his hands. Even when he went to the back for whatever reason as he was cooking, he NEVER washed his hands or dried them cause his hands were wet when he came from the back!!!!! Food looks good but I’m turned off because of this. They have an A on the door however this was TOTALLY unsanitary. FYI: I made this review while waiting for my food cause I watched it all thru the glass. Not trying to sound super clean because the inside is very clean however this turned me off! Idk if I’ll come back after this.

Intentional Foods (IF Cafe & Market) – Menu with Prices – 1837 W Guadalupe Rd #103, Mesa

First timers… I have no food allergies and think their food is amazing. I never thought allergen free food would be so good! Let me just say this establishment deserves more than five stars. From the moment we walked in we just knew this was going to be a great experience. The place is clean! I mean from top to bottom. No dust or fingerprints. And they have a sink with soap out in the open… for anyone and everyone! Washing is encouraged. So why did we go? GREAT QUESTION! Obviously, we were hungry. LOL My wife is celiac and finding any place to eat that wont result in her being bed ridden is a gamble for sure and most often means we eat at home. There are so many pluses to eat at IF. If you or anyone you know has food allergies and feels like they can never go out to eat, pay a visit to IF. You just might have an emotional moment. Watching my wife light up and seeing her so happy while confidently enjoying her lunch was awesome! And in some way a relief. Lisa and Ned are wonderful people. They take the time to explain what goes into the prep of the food, the nutrients, and the back story as to why IF came to be. Their menu offers so many delicous options. The flavor of their food is so good. Portions are spot on. Temperature is key and is perfect when it hits the table. Their scones are incredible! The sourdough bread, THE BEST! If you have a sweet tooth, they’ve got you covered. This is a MUST GO TO place for sure! Thank you Lisa and Ned for creating “Intentional Foods”.

Metro Halal Food Cart Menu with Prices 1687 N Lynn St, Arlington

The food was pretty good and the owner is friendly but my big complaint is from a food safety perspective. The guy has a glove on one hand and nothing on the other hand. He then handles cash using both hands, or handles his cell phone for Square payments…and then uses both hands for shredding lettuce (by hand) and serving the meats/sides (with a spoon). Admittedly, I’m a bit of a germophobe but what’s the point of wearing a glove if you’re just going to handle cash/cell phones/etc. and then prepare and serve people food without switching gloves?? If he cleans up his food handling practices I’d probably change this to a 4 or 5 star review.

Vina Pho & Grill – Menu with Prices – 1630 30th St, Boulder

I’ll never be back. The only good things I can say about this place are: location, parking, and their service First time coming here last night. Place was half full for 6pm on a Wed.. Extensive menu, gave me high hopes. However, most everything has soy in it, so I can’t eat it. Ended up just ordering some Thai Spring Rolls, but they were SO boring, not authentic at all. I only ate half the order and shoved the plate away. I just couldn’t swallow anymore of it. My waiter explained the new-ish management changed things last year. No mint in the Spring Rolls anymore, because, he said, the kitchen had keep throwing it out – customers didn’t like the mint? I suspect it was crappy mint. So there was almost nothing in my spring rolls, just rice noodles, a bit of lettuce, and some bean sprouts. Dry and smelly. I opted not to try the peanut sauce it came with, as peanuts are not making my happy anymore either. unfortunately, the Sweet Chilies Sauce was weak in flavor and too watery. I decided if they cheap out on something as basic as their Spring Rolls, pretty sure the rest of the dishes are missing ingredients too. Cheap napkins fall apart as well. Odd bathroom setup. Nice to have a sink outside them, to just wash hands, but they only had gross, cheap, liquid hand soap that is full of chemicals and smells overly strongly too. My skin itched after using it. Had to keep rinsing them and then dug out my hand lotion to sooth my irritated skin. The 2 stars are for the great service only. I miss really good Asian food. I can’t stomach most of it anymore though, due to all the soy they all use.

Nola Bistro – Menu with Prices – 603 N Waverly Rd, Lansing

Staff greeted and seated us eventually. At no point did we feel welcomed. The aesthetic is modern. The floors were slippery beyond belief. I asked them if they use degreaser on the floor. She told me that she will tell the manager about degreaser. She said that the floor is slippery because it was raining. It was evident that the floors are slippery due to the lack of degreaser being used to clean the floors. The entire restaurant as well as bathroom were so slippery that I was worried about other customers falling. We actually moved across the floor in ice skating fashion. Partly because it was hilarious and partly for safety. The menu had veg based options so we were excited. The waitress had limited knowledge about the menu and/or ingredients on the menu. After receiving our pho I knew the broth was reconstituted I knew that Nola was basic. The noodles and accompanying small amount of veg were nicely cooked. The dish was topped with fresh herbs lovely, fresh herbs we’ve all grown to expect at standard pho type establishments. We asked for an additional side of herbs which we were willing to pay for. The accommodation was I met. Suddenly the relationship seemed to take a dark turn after the request. it seemed like a wonderful opportunity for an establishment to rise to the occasion. Unfortunately it had the opposite effect. The fresh rolls were basic salad & noodles with prefabricated goopy sweet sour sauce. If you are just looking to fill up, you can do that. If you’re looking for something a little more exciting, drive by. It’s like if MacDonald’s started a pho restaurant, basic. Upon our departure we waited at the counter to speak to someone about the overall safety concerns of the restaurant. After several minutes it was obvious that the overall experience is reflected in the management care and concern for their patrons.

i’ve been here about 4-5 times after my prior review. Man I didn’t want to have to write this but I feel like I need to for this business to get better, because clearly there seems to be some procedural issues at front of house. They have good food, but their customer service or maybe its just the one older guy at front of house that needs to be replaced with someone younger and hungrier to grow the business by meeting basic customer needs in the to go category. start of with the pro’s pro – their trio plate is good if all things are warmed to the right temperature and given in the right portions (really only just 2 small meatballs in a trio plate? , you clearly have room for 4, 2 is being extremely cheap) Go with biryani and butter chicken, if they are heated to the correct temperatures you will enjoy those flavors. pro – they have improved their system of not putting the trio food selection out, because it would dry out. the chow mein was hard when I had it the last couple of times, and I am glad they have identified this quality issue. con – I think they basically just leave their food cold somewhere and then reheat as necessary, which is evidenced by the consistency of the cold or not even lukewarm food i keep getting from them. ——– i’ve ordered to go here, and for the first time in 25 years of ordering food from restaurants I was given a room temperature unwarmed entree to take home, and no utencils or paper towel in the to go bag. The older guy didn’t even bother to get it warmed up, any to go dish that is a warm entree should be cooked before allowing a customer to take it home. This to me speaks volumes about lazyness and negligence of customer service of this gentleman, I just don’t think he has any business sense in the customer service department and has no business manning the front area. This is a very basic customer service routine. I was pretty insulted by this and had thought this was the last time I would go to this place. fast forward 3 months, I revisit after a workout to see if things had improved. I order a trio dish and veggie samosa with no drink TO GO. (again no utencils packed, when it is clearly near the counter and no paper towels) 14$ chicken briyani and butter chicken quality is still excellent, however i ordered meatballs and i only received TWO lol, and there was plenty of room for 2 more, this is just being cheap AND the meatballs were not WARMed up. Come on man, I don’t want to get ecoli or whatever from cold meat hanging out.. who knows how long its been SITTING out there. Just straight killing me mentally. The veggie samosas were hot on the outside and still cold on the inside. This just speaks to not having proper procedures setup in the kitchen on reheating. I feel like this business could take really take off, as they do have certain dishes that cooked very well and tasty. If it were managed properly by people who are familiar with customer service basic needs and energy and passion to run the business properly. My last 3-4 times have just been tainted with repeated lazy and cheap customer service experiences. I wish I could just steal the cook away and start my own business with them. You need to work harder man, I want this place to succeed but I just can’t keep wasting my hard earned money with all these issues every time i eat there.

Tim Hortons – Menu with Prices – 10221 170 St NW Unit 1020, Edmonton

Close to my work, so I go here. Some of the staff members are not doing their job nicely. Last month I went to buy chicken panini, a Filipino lady was washing dishes, then she without washing her hands made my sandwich without even using gloves. When I refused to take it, she did not accept her mistake. When I refused again, she made very weird face, but made me new sandwich. Just a tip, if anyone goes to this Tim hortons, make sure you keep an eye, if they are making with clean hands.
